Project Brief
Design a care facility intended to support and complement the services offered by the existing nearby medical centre. The proposed scheme should act to provide “homes for life” for elderly people with on-going nursing care needs.
Following the Graham Care Development model of “Relationship centred care”, the proposal should enable a situation in which carers can build relationships with the patients family and friends, in addition to their traditional relationship with residents.
Designated as one of the 12 Vanguard sites in the UK, the NHS are working jointly on this project to consider new ways of delivering care to meet the growing demand.
Outcome
The proposed suites provide residents with much more space than a standard nursing home. These larger suites include a kitchenette and social space with seating so that residents’ families and friends can spend quality private time together in privacy and comfort. A dining space gives residents the choice to eat in the privacy of their own room, if they wish.
The integrated community healthcare centre provides capacity to care for complex patients without the need for A&E / acute admission. This facility will be available to all GP’s and care homes in the local area with enhanced proactive community medical input.
